All inquiries, complaints and suggestions from patients or users of laboratory services can be made in writing / orally by using the relevant form, email or telephone to the Laboratory Coordinator or Customer Feedback Officer, Oral Pathology Laboratory as follows:

Users of laboratory services must fill in the Customer Feedback Form (HPUSM/LCD/QP-08/R1) obtained from the Oral Pathology Laboratory. The form must be returned to the laboratory for service improvement and documentation. Information regarding the actions taken by the laboratory on all inquiries, complaints and suggestions will only be given to the complainant who provides complete details. Any inquiries, complaints and suggestions without identification will not be processed.

All feedback and customer satisfaction surveys are appreciated and action will be taken according to the established procedures (Customer Feedback Management and Customer Satisfaction Survey - HPUSM/LCD/QP-08).
